Aspectos psicológicos diferenciales en enfermos con insuficiencia renal crónica en función del método de tratamiento. Un estudio piloto

[Psychological aspects in patients with chronic renal failure with regard to dialysis treatment. A pilot study]

Resumen

En este estudio se analizan las diferencias en los niveles de ansiedad, depresión, percepción subjetiva de salud e indicadores de salud objetiva, entre dos grupos de enfermos renales, sometidos a Diálisis Peritoneal Continua Ambulatoria (D.P.C.A.) y Hemodiálisis Hospitalaria (H.D.H.) respectivamente. Un total de 22 sujetos voluntarios, con una edad media de 51 años, respondieron al cuestionario de Ansiedad de Estado-Rasgo de Spielberger, al Inventario de Depresión de Beck y al Perfil de Salud de Nottingham (percepción subjetiva de la salud). Como indicadores de salud objetiva, utilizamos los resultados del último análisis clínico, las complicaciones derivadas de la técnica, las derivadas de la uremia y la, presencia de síntomas interdiálisis. No encontramos diferencias estadísticamente significativas entre grupos ni en ansiedad ni en depresión. Sin embargo, los enfermos sometidos a D.P.C.A. obtuvieron puntuaciones significativamente más favorables que los sometidos a H.D.H. en las siguientes dimensiones de salud percibida: movilidad, sueño, dolor y energía, así como en algunos indicadores de salud objetiva. Se discuten estos hallazgos en relación con los resultados procedentes de otros investigaciones

Abstract

In this article we analyse the differences in the levels of anxiety, depression, subjective health perception and objective health indicators, between two renal patient groups undergoing Continuous Ambulatory Peritoneal Dialysis (C.A. P. D.) and Hospital Hemodialysis (H.H.D), respectively. A total of 22 volunteer subjects, whose mean age was 51, answered the Spielberger State Trait Anxiety Inventory, the Beck Depression Inventory and the Nottingham Health Profile (health self awareness). As objective health indicators we used the results of the last clinical analysis, the complications associated with the technique, the derivatives of the uremic syndrome and the presence of intradialysis symptoms. We didn't find statistically significant differences between the groups, in anxiety and depression. However, the patients undergoing C.A.P.D. had statistically significantly better scores than the patients undergoing H.H.D. in the following self awareness dimensions: mobility, sleep, pain and energy as well as some objective health indicators. We shall compare these results with others obtained from previous investigations
